Patient Portal

Our patient portal allows you to update your personal and health information before your visit to save time. If you cannot complete your portal before your visit, please arrive 30 minutes before to complete it. You must first call the office to receive a portal login. You will be redirected to Saido for the portal.

New Patients

 If you prefer not to use the patient portal, you may print and fill out the paperwork below. We appreciate you completing this BEFORE you come for your appointment. You may also fill out and email to [email protected] to save time at check-in.

We are approved on most major commercial insurance carriers, including BCBS, Aetna, Cigna, Humana, United Healthcare, as well as Medicare. If your insurance is not listed, please call our office (318) 933-3376.

Cancellation Policy

CenLa Dermatology strives to see all patients in a timely and efficient matter. As a courtesy, we attempt to contact each patient 24 to 48 hours prior to their scheduled appointment to remind them of the date and time. However, it is the responsibility of the patient to arrive on time for their appointment. Your time is valuable, as is ours, and if you are unable to keep your reservation, we request that you give our office a minimum of 24 HOURS notice, should you need to cancel or reschedule an appointment. If this is a recurrent problem, you will be asked to pay a deposit to book your appointments.  we appreciate you working with us to be able to offer adequate booking options.
